The health of women and children is the foundation of national health. In recent years, the provincial party committee and government have adhered to the people-centered work orientation, and carried out the "two cancers screening" for women free of charge throughout the province for many years, and improved the mechanism of prenatal screening and neonatal disease screening. This is not only an important measure to improve the health protection level of women and children, but also a popular project that benefits millions of families. I am very glad to meet you here! I would like to take this opportunity, on behalf of the Provincial Health and Wellness Committee, to extend a warm welcome to friends in the press! I would like to express my heartfelt thanks to the news media and all walks of life who have long cared for and supported the development of health care, especially maternal and child health. Below, I will focus on three aspects to introduce the implementation of people’s livelihood facts (hereinafter referred to as free screening of people’s livelihood facts) to improve the health protection level of women and children in the province.
I. Basic information
The provincial party committee and government attach great importance to improving the level of women’s and children’s health protection, and will continue to incorporate this work into key livelihood issues in 2022. The Provincial Health and Wellness Committee, in conjunction with the Provincial Department of Finance, the Civil Affairs Department, the Women’s Federation, the Disabled Persons’ Federation and other relevant responsible units, coordinated the prevention and control of the epidemic situation in COVID-19 and focused on the practical work of people’s livelihood, carefully organized and solidly promoted, and the province’s free screening of people’s livelihood was completed as scheduled.
By December 31, 2022, the province had completed 842,600 free screenings for cervical cancer and 860,100 free screenings for breast cancer, achieving 105.3% and 107.5% of the annual target tasks respectively. 576,700 cases of free prenatal ultrasound screening and 541,600 cases of free serological prenatal screening were carried out, and the coverage rate of free prenatal screening for pregnant women reached 73.5%, 13.5 percentage points higher than the provincial target task; 775,800 cases of neonatal "two diseases" (phenylketonuria (〔PKU〕) and congenital hypothyroidism (〔CH〕)) and 774,500 cases of hearing screening were completed, with screening rates of 98.9% and 98.7% respectively, which were 0.9 and 0.7 percentage points higher than the annual target tasks. All the provincial cities and Jiyuan demonstration areas in the province have exceeded the provincial target tasks.
Second, the main promotion measures
During the implementation of the project, the whole province made concerted efforts and the relevant departments cooperated closely to overcome the impact of the COVID-19 epidemic and jointly promote the effective and orderly development of this livelihood.
(1) Strengthen organizational leadership. The provincial party committee and the provincial government have listened to reports on special topics for many times and kept abreast of the progress on a regular basis. The general office of the provincial government issued the Measures for Practical Assessment of Key People’s Livelihood in Henan Province in 2022, and established a systematic assessment system such as supervision and inspection, quarterly notification, mid-year evaluation, self-examination at the end of the year, verification and identification, and year-end assessment; Provincial Development and Reform Commission monthly data monitoring and work supervision on the progress, quarterly summary of the progress of the provincial government, and through the provincial government website for notification; The Provincial Bureau of Statistics and the Development and Reform Commission jointly issued the "Implementation Measures for the Verification and Confirmation of Key People’s Livelihood Facts in Henan Province in 2022" to standardize the verification and acceptance of the completion of people’s livelihood facts, and organize the masses to conduct satisfaction surveys and evaluations on the implementation of people’s livelihood facts; The Office of the Provincial Leading Group for Free Screening of People’s Livelihood Facts adheres to the quarterly notification system, and informs the relevant departments of local governments about the completion of tasks every quarter, reminding and urging areas with slow progress to intensify their work. The above provides a strong organizational guarantee, institutional guarantee and mechanism guarantee for the smooth implementation of free screening of people’s livelihood.
(2) departments work together to promote. The Provincial Health and Wellness Committee gives full play to the leading role of the main responsible units, summarizes work experience, studies improvement measures, formulates work plans in conjunction with relevant responsible units, further clarifies objectives and tasks, divides responsibilities, refines work measures, arranges and deploys the work of free screening of people’s livelihood in the province, and promotes the implementation of tasks; The Provincial Department of Finance allocated 276 million yuan at the provincial level, and urged all localities to implement matching funds to provide financial guarantee for the smooth implementation of people’s livelihood; The Provincial Women’s Federation continued to publicize and launch, organized online contests on core knowledge, publicized the policy of benefiting the people, and improved the initiative and compliance of mass participation; The Provincial Department of Civil Affairs conducted an accurate survey to provide timely information on school-age women screened for "two cancers"; The Provincial Disabled Persons’ Federation optimizes rehabilitation measures and gives priority to providing rehabilitation measures such as cochlear implant for children with hearing impairment diagnosed by screening. All responsible departments have worked closely together to form a strong joint force to promote the work.
(3) Coordinating epidemic prevention and control. The provincial party committee and government attached great importance to the work of maternal and infant safety during the COVID-19 epidemic, and set up a "special class for women and children" when adjusting the organizational structure of the provincial epidemic prevention and control headquarters. Provincial leaders have repeatedly stressed in the provincial epidemic prevention and control headquarters and grassroots research and guidance that while doing a good job in COVID-19 epidemic prevention and control, they should carefully organize and scientifically arrange to ensure that the free screening of people’s livelihood is effective; City and county governments should strengthen their responsibilities, study measures to promote epidemic prevention and control and people’s livelihood, increase publicity, improve people’s awareness rate, create a good atmosphere, and organize and guide clients to actively participate in screening; The province’s health system obeys the overall situation of epidemic prevention and control, formulates different coping strategies according to the different characteristics of "two screens" and "two cancers" screening, opens up green channels, and implements peak-shifting screening, thus ensuring the overall consideration and coordinated promotion of epidemic prevention and control and practical work of people’s livelihood.
(4) Strengthen quality management. Taking the opportunity of in-depth implementation of the "Year of Ability and Style Construction", the health and health system continued to carry out personnel training, and trained 2706 professional and technical personnel to screen people’s livelihood for free throughout the year, with certificates and standardized services; Adhere to the online and offline quality control system of provincial experts, conduct online quality control every month, conduct on-site research and guidance once every six months, and provide targeted guidance when problems are found; Adhere to the provincial monthly meeting system, report the work progress and quality control situation in various places every month, study and solve existing problems, and continue to promote the implementation of the work; Optimize and upgrade the province’s maternal and child health and livelihood information system to make the work more efficient, the service more standardized and the data more objective.
Third, the main work results
In 2022, through the free "two cancers" screening, 264 cases of various types of cervical cancer and 2000 cases of precancerous lesions were diagnosed in the province; 409 cases of breast cancer and 10 cases of precancerous lesions were diagnosed. All the above cases have been treated by timely and standardized medical treatment or referral treatment.
Through free prenatal ultrasound screening, 5797 cases of high-risk abnormal pregnant women (fetuses) were found in the province; A total of 32,455 high-risk abnormal pregnant women were found through free prenatal serological screening. Through free prenatal diagnosis of 16768 cases, a total of 1587 children with chromosome diseases such as trisomy 21 syndrome or monogenic diseases were diagnosed. The above-mentioned high-risk pregnant women and fetuses with severe birth defects have all received standardized medical treatment.
Through neonatal disease screening, 128 children with phenylketonuria (PKU), 539 children with hypothyroidism (CH) and 427 children with congenital hearing impairment were diagnosed in the province. Most of the above-mentioned children received timely and standardized treatment.
In 2023, free screening has been included in the provincial key livelihood issues for seven consecutive years. Since 2017, our province has included free screening in the key livelihood issues of the province for many years, and expanded the service content year by year, which fully reflects the great attention and care of the provincial party committee and the provincial government for the health of women and children. Qi Xin, the relevant departments of the whole province, made concerted efforts and worked hard, exceeding the provincial targets and tasks every year, and achieved the effect of satisfying the government and benefiting the masses.
First, strict organization and personnel management. All institutions involved in providing services are qualified professional technical service institutions recognized by the health administrative department, and technical personnel providing services must undergo standardized training and hold certificates.
The second is to establish a perfect service system. The province has built three systems to support the effective operation of the project from top to bottom, namely, the technical service system with medical and health institutions at all levels undertaking screening tasks as the main body, the project management system with provincial, municipal and county health administrative departments and maternal and child health institutions as the main body, and the information support system with the unified maternal and child health information data platform as the main body.
Third, adhere to strict quality control system. Focusing on the image quality of prenatal ultrasound screening, colposcopy for cervical cancer screening, B-ultrasound for breast cancer screening and other key links, provincial and municipal experts can use the provincial maternal and child health information system in their own clinics to conduct quality control and technical guidance for service institutions in their jurisdictions, so as to achieve full coverage of online quality control of service institutions. Organize on-site quality control guidance by provincial experts to ensure full coverage of counties (cities, districts) every year.
The fourth is to continuously improve the whole chain service. Free prenatal diagnosis and follow-up for high-risk pregnant women with prenatal serological screening have been carried out, and 111,000 cases have been diagnosed, and 9,066 children with chromosome diseases such as trisomy 21 and monogenic diseases have been diagnosed, all of which have been treated by standardized medical treatment; Diagnose the suspicious children who were screened for "two diseases" of newborns, and 1165 children with phenylketonuria and 4516 children with hypothyroidism were diagnosed, all of which were transferred to the provincial neonatal disease screening center for follow-up intervention treatment; High-risk children with free newborn hearing screening were diagnosed, and 3011 children with congenital hearing impairment were diagnosed, and the disabled persons’ Federation departments were coordinated to give priority to rehabilitation assistance.
Through the continuous implementation of free screening of people’s livelihood, urban and rural residents’ awareness of eugenics and health has been continuously improved, and more and more people have changed from "I want to screen" to "I want to screen". The coverage rate of prenatal screening, the screening rate of neonatal "two diseases" and the screening rate of neonatal hearing increased from 20.79%, 88% and 77% in 2016 to 73.5%, 98.9% and 98.7% in 2022 respectively, which was better than the national average.

In order to support this livelihood, the financial departments at all levels in the province have mainly done three tasks:
First, arrange the budget funds in full. In 2022, when the contradiction between revenue and expenditure was prominent, the financial departments at all levels in the province continued to optimize the expenditure structure and tried every means to raise 600 million yuan, including 276 million yuan at the provincial level, which effectively guaranteed the smooth development of various screening work.
The second is to speed up the disbursement of funds. In order to improve the efficiency of the use of fiscal funds, we issue some subsidy funds in advance every year, set up a ledger of special funds in place in the middle of the year, urge cities and counties to allocate them in time and in full through monthly reports, and then settle the special funds according to the facts at the end of the year to ensure that the fiscal funds are actually allocated in place.
The third is to improve the fund management methods. In 2022, we studied and revised the Measures for the Administration of Five Subsidies for Basic Public Health Services, and put forward specific requirements for the arrangement, distribution and release of special funds for "two screening" and "two cancer screening" to further improve the standardization and safety of the use of funds.

First, focus on publicity and promotion. The Provincial Women’s Federation held a publicity campaign on "Healthy Central Plains’ Action", "Two Cancers" and "Two Screens" and a prize-winning contest on women’s health knowledge on WeChat WeChat official account, which mobilized nearly 50,000 women in the province to visit and more than 36,000 people participated in the answer sheet, which effectively publicized the practical policies of "Two Cancers" and "Two Screens" and further popularized women’s health knowledge. In 2022, women accounted for 49% of the urban subsistence allowances in the province. Civil affairs departments give full play to their functions, guide cities and counties to cooperate with the propaganda, organization and inspection work of "two cancers" screening for people’s livelihood, provide timely lists and basic information of urban minimum living allowance women of school age, and promote the smooth development of this people’s livelihood work.
The first is to optimize the workflow. In the name of the general office of the provincial government, the implementation opinions on strengthening the dynamic monitoring and assistance of low-income population were issued, the measures for identifying low-income population were improved, and the hierarchical and classified social assistance system was accelerated. We will deepen the reform of aiding streamline administration, delegate power, strengthen regulation and improve services, formulate the Measures for the Examination and Confirmation of Minimum Living Security in Henan Province, delegate the examination and confirmation of minimum living security to towns (streets), shorten the time limit for handling, and make it convenient for people in need.

The first is to strengthen publicity and guidance. Make full use of time nodes such as Love Your Ear Day, Helping the Disabled Day and Disability Prevention Day, focus on key publicity contents, give play to the role of traditional media and new media, and extensively carry out popular science activities with distinct themes and various forms, such as "caring for hearing health and listening to a wonderful future" and "popularizing disability prevention knowledge and building a healthy China". The second is to strengthen information sharing. The Provincial Disabled Persons’ Federation and the Provincial Health and Wellness Committee establish a working cooperation mechanism, share information regularly, register and manage the screened suspected disabled children in a timely manner, specify the person responsible for docking, inform the rescue policy, application procedures, contact information, etc., so that guardians can truly know the policies, standards and ways, apply for rehabilitation assistance according to their own conditions, and realize the work goals of early detection, early diagnosis and early intervention.
The third is to promote disability prevention. Leading the formulation and implementation of the new disability prevention action plan, and establishing the Henan Provincial Disability Prevention Coordination Group and the Disability Prevention Expert Advisory Committee. Combined with the construction of prenatal screening and prenatal diagnosis system in the province, we will implement dynamic management, optimize and adjust screening and diagnosis service institutions, and further improve the quality of screening and diagnosis services.
The third is to continuously optimize the screening program. In terms of free "two cancers", we are about to start a new cycle of screening work.
