The examination found that the patient’s neck and back were symmetrically raised in many places, and it felt like soft fat lumps instead of strong muscles. After inquiring about the medical history, the patient had a history of drinking for 30 years and drank about 500 ml of rice wine every day. In the past 10 years, there have been many painless lumps in the neck and back, which have increased year by year. Xu Chunhua, deputy director and deputy chief physician of the Second Ward of Infectious Diseases Center, checked the patient’s condition, combined with his B-ultrasound and other auxiliary examinations, and made it clear that the patient had a rare disease — — Ma Delong’s syndrome.
Xu Chunhua explained that Ma Delong’s syndrome is a hidden and slowly progressive benign tumor disease, also known as benign symmetric lipoma. Fat is diffused in the upper part of the body, especially in the neck, nape and back, showing the appearance of "hippopotamus neck" and "hump back". Most of the patients are middle-aged men, and the onset is closely related to long-term heavy alcohol intake. The main clinical manifestations are painless fat lumps symmetrically distributed in the bilateral neck, occipital region, nape region, shoulder region, submandibular region, suprasternal fossa and supraclavicular fossa, which are similar to tumor lesions, often lead to neck deformity, limited movement, and a "cow neck" or "hump back" appearance, which affects life and appearance.
With the growth of the fat lump in the neck, it will not only lead to the restriction of the neck movement, but also cause severe compression of the trachea and esophagus, causing breath holding, dyspnea, dysphagia, and even compression of the vena cava, resulting in venous reflux disorder. Ma Delong syndrome is often associated with impaired glucose tolerance or diabetes, hyperuricemia, anemia, hypothyroidism, endocrine tumors and other diseases.
At present, surgical resection is an effective treatment method to improve deformity and solve dysfunction. Liposuction is also used as a treatment, but the fat tissue of this disease is tough, so it is difficult to fully suck off the fat, and the postoperative recurrence rate increases. Conservative treatment in internal medicine is mainly abstinence from alcohol, such as alcoholic liver disease, diabetes, hyperuricemia, etc., and comprehensive treatment should be carried out to protect liver, reduce blood sugar and uric acid.
Xu Chunhua reminded that long-term heavy drinking may not only lead to alcoholic hepatitis, liver cirrhosis and even liver cancer, but also affect fat metabolism, leading to multiple organ damage in the heart, brain and kidney. Therefore, we should establish a correct concept of drinking, drink less or not, especially don’t drink too much.
